VISION STATEMENT • Our Vision is for our diverse learners to be empowered, prepared and equipped with academic and life skills needed to be productive citizens, who can positively contribute to society. • Our Goal is for every student to have clear attainable pathways to career and/or college success. In Support of our Vision , we will attract and retain highly qualified staff, as well as, leverage community resources to provide support systems for all students and their families. MISSION STATEMENT • Our Mission is to meet or exceed our goals in the area of equity, excellence, educational effectivenes s and economic sustainability which determines our daily efforts. We will foster collaborative teamwork to accomplish these goals. We recognize that we must build powerful home to school relationships that support the success of all students. Because safety is critical for each school campus; we will enforce high adult visibility and respond to student matters in a timely manner. In order to assess whether students are meeting academic and social expectations, we will consistently monitor student progress using a variety of data: observations, walkthroughs, progress reports, test scores, etc., and commit to focusing our resources to best support our students. IMMUNIZATION INFORMATION California law requires schools to maintain the privacy of students’ health information. Your child’s identity will not be associated with any report produced as a result of the requirements. If you have questions about the State’s health requirements, please contact the District’s Full Service Community Schools office at 707-556-8921 ext. 50142. California State Law requires that all students be properly immunized. Parents or guardians of students in any school or child-care facility, whether public or private, will no longer be allowed to submit a personal or religious beliefs exemption to a currently-required vaccine. The immunization requirements do not prohibit pupils from accessing special education and related services required by their individualized education programs.

PARENTS’ GUIDE TO IMMUNIZATIONS REQUIRED FOR SCHOOL ENTRY Starting July 1, 2019 Students Admitted at TK/K-12 Need: • Diphtheria, Tetanus, and Pertussis (DTaP, DTP, Tdap, or Td) — 5 doses (4 doses OK if one was given on or after 4th birthday. 3 doses OK if one was given on or after 7th birthday.) For 7th-12th graders, at least 1 dose of pertussis-containing vaccine is required on or after 7th birthday. • Polio (OPV or IPV) — 4 doses (3 doses OK if one was given on or after 4th birthday) • Hepatitis B — 3 doses (Not required for 7th grade entry) • Measles, Mumps, and Rubella (MMR) — 2 doses (Both given on or after 1st birthday) • Varicella (Chickenpox) — 2 doses These immunization requirements apply to new admissions and transfers for all grades, including transitional kindergarten. Students Starting 7th Grade Need: • Tetanus, Diphtheria, Pertussis (Tdap) —1 dose (Whooping cough booster usually given at 11 years and up) • Varicella (Chickenpox) — 2 doses (Usually given at ages 12 months and 4-6 years) In addition, the TK/K-12 immunization requirements apply to 7th graders who: • previously had a valid personal beliefs exemption filed before 2016 upon entry between TK/Kindergarten and 6th grade • are new admissions Records: California schools are required to check immunization records for all new student admissions at TK/Kindergarten through 12th grade and all students advancing to 7th grade before entry. Adam Clark, Ed.D. • Superintendent Dear Parents/Guardians: Re: Policy and Procedures Regarding Students Taking Medication at School If your child needs to take prescription or non-prescription medication at school, please get the “SCHOOL FORM – AUTHORIZATION FOR MEDICATION REQUIRED DURING SCHOOL HOURS” completed by the Authorized Healthcare Provider. This is required for both prescription and non-prescription drugs. This form may be obtained from the office manager/secretary or the school nurse. A STUDENT’S MEDICATION FORM IS REQUIRED TO BE RENEWED AT LEAST ONCE A YEAR. Once the form has been completed it should be brought to the school office along with the medication. Prescription medication information must be on the label: Student’s name, Authorized Healthcare Provider’s name, name of medication, dosage, method, and date medication was prescribed and expiration date. Non-prescription medication (i.e., cough drops, Tylenol, decongestants) must be in their original containers which are clearly labeled with student’s name, name of medication, dosage, method and expiration date if applicable. Designated School Personnel can assist a student in taking medication as ordered by the Authorized Healthcare Provider. Students may carry medication and self-administer when they have a “SCHOOL FORMAUTHORIZATION FOR MEDICATION REQUIRED DURING SCHOOL HOURS”, in addition to a “Medication Self-Administration Form” completed and on file in the school office. PLEASE NOTE: Medication should be delivered to the school by a parent/guardian or adult designee and should also be picked up by the adult at the end of the school year, with the exceptions of medication(s) a student is authorized to self-administer per Authorized Healthcare Provider’s orders. MEDICATION WILL NOT BE SENT HOME WITH STUDENTS. Any medication not picked up will be discarded.
