welcome to a new year of faith formation for the children of st mark parish our program is designed for youth in pre-k through grade 6 days times and gathering place gatherings are held on the following days faith formation sunday mornings english mass 9:30a class time 10:40a 11:20a praise worship/fellowship 11:30a or spanish mass 11:30a sacramental prep wednesday evenings 6:30p 7:30p c.l.w children s liturgy of the word during 9:30 a sunday mass gatherings are held in the st mark parish center about the program pre-k through 6th graders will be growing in faith through bi-monthly faith formation classes and periodic family sundays each sunday gathering includes praise and worship bible stories arts and crafts snack and exploration of our rich catholic traditions and beliefs we use texts and materials approved by the archdiocese to teach the catholic christian faith which includes the scriptures catechism of the catholic church church history saints holy days and seasons of the liturgical year church documents prayer sacraments and social justice program fees faith formation $50 for 1st child $25 each additional child sacramental prep $40 book fee includes two books page 3

be involved your participation in the faith formation classes of your child is very important to the success of the program and importantly to your child s spiritual development you are welcome to come and join the fun and find ways to in which to help volunteers can commit to the entire year of gatherings or for specific events become a catechist is god calling you to lead young people then faith formation can use your talents you could be co-teaching helping behind the scenes putting your tech savvy skills to work or making sure our kids are safe contact mariah harris for more information 541.689.1054 supplies registration costs pay for basic supplies needed in the program as our program evolves we will have times when we require certain items families will be asked to donate items when needed and donations of basic supplies are always welcome plenty of notice will be given snacks parents are asked check out the snack wish list pg 8 please choose any items of your liking and we will use them for snack through out the year when items run out the wish list will be sent out again thank you for providing healthy and nutritious snacks communication in an effort to reduce paper waste information will primarily be shared through emails the guardian and our parish website www.saintmarkeugene.org the guardian for weekly faith formation youth ministry news every week you can find it in the bulletin on the parish website or sign up to receive it by email when you fill out your registration forms please leave a legable email address page 4

keeping our children safe drop off and pick up procedures in order to ensure your child s safety parents are requested to walk their children into the parish center sign their children in and return in the same manner to pick up their children behavior expectations all children are expected to do their best be respectful of themselves others and the facility work as a team glorify god in their words and actions if a child persists in not adhering to the expectations faith formation teachers and volunteers will first speak to the child and work together to develop an action plan for behavior improvement parents will be notified about this plan and asked to participate called to protecttm we can t be with our children at all times but we can learn ways to help keep them safe when we re not there we invite you to join us for the educational session called to protecttm for parents and families the 1.5 hour program will teach parents and family members how to help prevent child sexual abuse called to protect tm was written and developed for the church by praesidium the national leader in abuse risk management if you are a parent grandparent aunt uncle or godparent we encourage you to attend the session you ll learn facts and myths about child sexual abuse how offenders gain access to children warning signs to look for in children or adults actions to take when you notice warning signs we also provide educational sessions for elementary youth teens and catechists the archdiocese of portland requires us to educate as many people young and old as we can if you do not want your child or teen to participate in these sessions you need to contact mariah harris for further information the schedule for all sessions will be provided in the fall page 5

1st reconciliation 1st communion sacramental preparation classes are on wednesday from 6:30-7:30pm they are offered in english and spanish this is a 2 year program year 1 preparation for 1st reconciliation year 2 preparation for 1st communion classes are open to 1st and 2nd grade students they will receive 1st communion in spring of 2013 requirements family must be registered in the parish the child must attend bi-monthly sunday faith formation classes sacramental prep classes must be attended regularly unless arrangements are made with mariah harris dre/cym parents need to attend sacramental prep classes registration forms baptismal certificates and book fee s are due before your child can receive their sacraments page 6

these are all the necessary forms you must fill out to register your children faith formation registration form listing your child up to 4 in the family please make sure to fill out each part of the form student/youth emergency information and procedure form for each child in the family each year the archdiocese requires that one form be submitted for each child parent info form we ask that parents get involved in the parish by lending your time and talents where you feel called by the spirit to serve photo permission slip we are able to post photos of our youth on the st mark website www.saintmarkeugene.org but we need parental permission to do so sacramental prep form 1st reconciliation communion for your child in the 1st 2nd grade or older that is ready to make their 1st communion and reconciliation parish registration form please fill out if you are not a registered member of st mark page 9
