level 01 no task is so important that we cannot take time to do it safely safety vision the focus of entergy corporation is the safe orderly restoration of service to its customers our vision is target zero accidents therefore we view every task through the lens of safety every employee contractor and agent of the company is critical to the achievement of success entergy 12 safety principles 1 2 all injuries are preventable safety is our fundamental most important core value people are the most critical element each person is responsible for his/her safety and for the safety of others 3 4 line organizations are responsible for safety safety leadership is critical at every level all operating exposures can be controlled effective hazard assessment prevents accidents and injuries 5 management is responsible for establishing a safe work environment and clear expectations for safety performance every employee contractor or agent of the company has the right to work safely 6 7 8 safety is good business prevention of accidents is good business management must audit performance safety performance is critical to our success our work is never so urgent or important that we cannot take time to do it safely safety always trumps speed 9 deficiencies must be corrected promptly when a safety question exists you are obligated to ask it 10 employees must be trained to safely perform all assigned tasks and accountable for applying these skills on the job employees should insist that safe work procedures and rules are followed 11 safety is a condition of employment safety attitude and mindset is a critical condition of employment 12 off-the-job safety is an important part of overall safety efforts an injury at home affects us at work too 2

level 02 entergy s cardinal rules are the foundation on which we operate entergy cardinal rules entergy s cardinal rules must be adhered to at all times the distribution and transmission business cardinal rules include but are not limited to the following which apply to all employees agents and contractors of the company intentional violation of these rules will result in serious consequences including termination 1 do not report to work or work under the influence of unauthorized drugs or alcohol in violation of company policy 2 do not knowingly provide false or intentionally incomplete documentation or information during a safety investigation 3 intervene and correct any cardinal rule violations by coworkers failure to intervene and correct the hazard and/or stop the activity shall result in discipline for the employee witnessing the violation as well as the employee committing the violation 4 report cardinal rule violations 5 report all occupational accidents and injuries immediately and do not hide accidents or injuries 6 do not engage in physical horseplay or fighting 7 follow appropriate business unit rules with respect to working on energized electrical equipment 8 follow appropriate business unit rules for confined space 9 wear appropriate lifesaving personal protective equipment e.g rubber gloves and sleeves when working on electrical equipment as specified by business unit safety rules 10 follow business unit grounding rules 11 follow switching tag-out and clearance rules 12 follow appropriate business unit cover-up rules when working on energized lines and/or equipment 13 follow fall protection rules 3

level 03 you don t have to get hurt to work here storm restoration osha standards are the minimum acceptable requirements entergy s safety and work rules impose a higher standard of safety awareness and performance than many companies this reference manual provides general information and guidelines for working on entergy s system and properties an entergy lead will be assigned to help coordinate your local activities upon your arrival at your work location the entergy lead will provide more specific details about your work assignments local information contacts expectations etc if there are conditions that are not completely understood work shall not begin under any circumstances please contact your entergy lead to discuss the task and its safe execution 4

level 04 view every task through the lens of safety emergency procedures contact information fill in the information below with the following contact information provided by your local entergy lead location/network name emergency local entergy entergy lead name entergy lead police ambulance procedure for serious accident or injury in case of a serious accident please use the following procedure 1 call 911 or the following emergency number 2 state the following a company name b crew foreman c nature of the accident d of persons injured e request emergency transport f stand by the radio or telephone to give added information g comfort and re-assure the injured 3 contact your entergy lead and/or local entergy office procedure for non-serious accident or injury 1 immediately contact your assigned entergy lead 2 transport the injured to the first aid station at the staging site 3 complete storm restoration accident reports basic first aid 1 entergy will provide you the best medical care available 2 proper personal hygiene can deter many medical conditions and illnesses please take care of yourself 3 proper care of minor cuts and scrapes will deter serious medical conditions please seek proper care so that the minor issues do not become serious conditions 4 all trips to medical facilities for treatment must be accompanied by your entergy lead or designee we expect to be notified at the onset of any potential injury or medical condition 5 proper hydration liquid intake and rest time is critical to avoid heat related illness please drink plenty of water and take 10 minute breaks at least every two hours 6 alcoholic beverages are strictly prohibited on entergy time and property see the appendix for emergency procedure pocket card and vehicle hangtags that will need to stay with you and in your vehicle at all times 5

level 06 staging areas traffic flow and control the maximum speed on entergy property is six mph unless designated otherwise pedestrians always have the right of way park in assigned areas seat belts are required during operation ride within the confines of the vehicle ­ not in the truck bed on the tailgate etc speed limit 6 forklifts forklift operators must be trained on the specific equipment seat belts are required during operation housekeeping this is your responsibility if you mess it up please clean it up pole piles do not climb on pole piles ground crew must maintain safe distance during loading please help us keep pole piles at a safe height/level proper attire and personal protective equipment hardh at safety glasses safety vest gloves boots appropriate puncture-resistant footwear shall be worn canvas recreational or open-toed shoes are prohibited shorts are prohibited during work duty approved dot reflective safety vests shall be worn in traffic flow areas hard hats and safety glasses shall be worn in material loading areas 7

level 07 safe driving entergy deploys the smith driving system for defensive driving the five keys of smith driving are aim high in steering get the big picture keep your eyes moving leave yourself an out make sure they see you other entergy driving tips include park so that you can pull forward upon departure avoid backing complete a safety circle walk before entering your vehicle if you must back use a flag person plan your route drive 5 mph below the posted speed limit treat intersections with inoperable traffic signals as a 4-way stop traffic control work zone protection traffic control devices including traffic cones signs reflective vests and flag persons shall be used in accordance with dot regulations all traffic control devices used shall be of standard design 8

level 09 when dealing with poison ivy or poison oak leaves of three leave them be poisonous insects animals plants there are a variety of poisonous plants snakes and insects in the entergy service area be aware dog bites hurt treat all snakes spiders and insects as if they are poisonous and avoid them always apply insect repellent such as buzz off or deet before going to the field mitigate the affects of poisonous plants by applying barrier creams such as spc zanfel and tecnu extreme are extremely effective poison ivy medications if rash or symptoms appear please apply these medications immediately 11

level 10 environmental stewardship environmental stewardship is a fundamental value at entergy we are determined to perform our work in an environmentally friendly manner consistent with federal state and local laws spills report all spills to your entergy lead immediately examples may include fuel spillage hydraulic oil or leaking equipment containment attempt to contain the spill using absorbent materials transport do not transport damaged or leaking transformers leave damaged or leaking equipment at the jobsite and report the location to your entergy lead for pick up chemicals shall shall shall shall have a material safety data sheet msds readily available be handled and used in compliance with the msds be in a labeled container be properly stored when not in use gas cylinders gas cylinders shall have the protective cap properly installed when not in use 13
