The Paradigm Shift in Healthcare by Dr Amir Hannan Dr Amir Hannan is named as one of the United Kingdoms Top 50 Clinical leaders by the prestigious Health Service Journal (HSJ) There is a very big change happening in healthcare – it’s called a paradigm shift. Thomas Kuhn coined the phrase defining it as “a change of thinking from one way to another” which is “driven by agents of change”. For thousands of years, when you felt ill or something wasn’t quite right, you would go and see your doctor. He or she would ask you a series of questions – some obvious, some less obvious, some deeply personal that even your closest lover knew nothing about. Your doctor would examine you, send you for some tests and investigations and then ask you to return so that he or she could then tell you the results when they could see you again. Literally overnight things are changing now so that you no longer have to wait. You can now see the same information as your clinician as soon as it is available. Why has this changed happened? The internet is now widely available. Latest figures from Ofcom shows 80% of the population now has access to broadband here in the UK. Notes are increasingly being recorded on IT systems electronically as we move away from paper-based systems. Patients are able to view what the doctor or nurse thinks and also see the results as soon as they are available. Patients can then think what this may mean before they see their clinician and determine together what choices they would like to do. By being involved in the decisions about their health, they are more likely to follow the choices they make. This can lead to better outcomes for the patient. As patients learn more about the choices they can make and what is available, they gain a better understanding of their health needs. The internet makes it easier for people to know what is happening by making knowledge more accessible. Who are the agents of change? Is it the doctor? Is it the nurse? Is it healthcare organisations or schools and colleges or the education system? It could be all those things but in my experience it is also a combination of the family, the community and society at large. We all are agents of change. Watch the Lego Movie if you don’t understand why! When we begin to feel we can do more for ourselves and that the power lies in us supported by the system and our clinicians then see how we transform ourselves. We either wait for things to happen or we start to ask “why can’t we begin to feel more in control and be active partners in our care”? In the next article, I will explore why consent matters and how we enable patients to safely cross the paradigm and sign up for the service. 10

The Gift of Family Love is better than any Present by Harry Singleton I live in a completely different world now and I am lucky enough to get some wonderful presents. Children now know about each other`s presents and naturally they boast about them to others In turn this puts their parents under enormous pressure to want their own children to have the same presents even if they cannot afford them. However the truth is that there is no shame in not being able to afford the latest fashions, computers or other modern gadgets. I may only have got my apple, orange, chocolates and modest set of games for Christmas but my parents encouraged me, loved me,did their very best for me and that was much more important than any Christmas present. Years from now children may not remember the presents you bought them but they will remember the love you gave them throughout the year. Wishing you all a peaceful Happy Christmas, HARRY I was probably about eight years of age and I was getting really excited because Christmas was imminent. My dad was an Engineer and worked long hours and we never seemed to have any money. I went to his firm`s Christmas Party and I lined up with the other children to receive my present. I got so excited when I received an Apple and an Orange. On Christmas day I woke early as my excitement had reached fever proportions and I couldn`t wait to open my 2 main presents. I was elated to open my first one, a small selection box of Chocolates Like any child I was eager to get onto my main present and I was not unhappy to find it was a Compendium of Games. Within minutes I was trying out the games in it. Snakes and Ladders, Ludo and drafts. I was so excited. 11

I’ve discovered that mostly, people get stuck in their lives, because of their ‘mindset’. “Our beliefs and past patterns become rigid and ‘real’, and as a result, we keep creating the same problems today that we had yesterday. It’s a vicious circle. The big secret to having a better life – better relationships, peach and happiness, more financial success, and better health – is to have a mindset of success. Our beliefs and old patterns and usual behaviors keep us in a rut of doing the same thing, Marcia at home

Marcia Martin making the same choices, and having the same results. We leave one job, just to find that in the new job, our old problems show up. We leave one relationship, and seem to find the same person the next time, and we have the same complaints and issues. How do we break the pattern? being right (about our belief ), or are we more interested in having a result in life that we actually want?” “Let go of your beliefs. Start to see the ‘possibility’. There are lots of possibilities. There are many ways we can look at a situation. One particular ‘mindset’ that works to provide new behaviours and new results, “Our old ‘mindset’ keeps us locked in place. is to consider that whatever is happening to We then get the same results, we’ve always you, is happening for a purpose for your wellhad. Just in a new circumstance. We need to being. Welcome the Experience, no matter ‘wake up’ and take a new stand. what it is. Consider what is happening in your life to be a ‘gift’, to teach you a lesson, to “First of all, consider that a ‘belief ’ is just provide you an experience, to give you a new that – it’s something you believe. It’s not understanding. Stop thinking of your life as necessarily ‘true’. But if you believe it, something that is ‘hard’, or ‘bad’, or ‘awful’; then you operate in a way that creates a circumstance that actually proves your belief and stop wishing it isn’t the way it is. Instead, start to welcome each and every experience to be ‘right’. People would rather be ‘right’ you have, as if it is a gift. You will be than happy. surprised how that ‘point of view’ will open up new choices, and new actions, and new “How we see the world – how the world ‘shows up’ for us, is totally dependent on our possibilities, that will begin to produce new results in your life. That will allow you the perspective, or our point of view. That’s why some people see the world as a ‘glass half full’, space to produce magic and miracles, instead and others see it as a ‘glass have empty’. The of always creating the same old problems and world is the same, but our perspective causes complaints. us to see either the possibility or the obstacle. “Another way to break up your old patterns We make our choices, determined by how and beliefs is to consider each moment, each the world shows up for us, which is a result experience and each person you encounter of our perspective in the first place, then we is NEW. We get stuck in thinking it’s going act, and then we have results. We have to to be the same as it was last time. We meet learn how to change our perspective. If we people in our lives, and already have an are thinking our ‘beliefs’ are ‘right’, it will expectation of how they will show up or act. be impossible to change our perspective. Actually that takes some commitment, and willingness to see things in a new way – even if we ‘believe’ person can’t show up any other way, than the way we create space for them to show up it is another way. We have to ask ourselves in. Our anticipation, and our expectations, the question, “Are we more interested in 13

actually create the room they have to express themselves. We expect the worst, and we are right. “Now that it is coming up on the Holidays there will be lots of time for being with family, and that will present it’s own problems. We get stuck in our old perceptions of our family members, because we have such long experience together, and it is hard sometimes to think of each connection as new, and we get caught up in our old perceptions of how they were the last time we saw them. Then, since we have that as our perception and we anticipate the same, we can only see them that way, no matter how they actually show up in the moment. Take this Holiday Time to practice having a new mindset of how the world is, and a new perception of what you expect from people. Make this Holiday Time a time of making amends, forgiveness, and gratitude. Practice being good to the people you love, and the strangers you meet. Anna’s Panorama wishes you all a wonderful Holiday and a Happy New Year! May 2016 bring you peace, love, happiness, health and prosperity. Sincerely, Anna Karin www.theannakarin.com

Think of what you are grateful for in life, instead of what is upsetting you, or what you are missing. Think of 5 things each day that you are grateful for, and write them down. Focus on that. Practicing gratitude will keep you in ‘present time’, instead of thinking about past regrets, or future fears. The practice of doing that, consistently each day, will cleanse your mind. You spirits will lift. You might even enjoy being with your family! Find a way you can give to others – volunteer at a homeless shelter, or go to a hospital and visit the old folks who are in hospice. Walk a pet from the animal shelter or visit a child who is ill. Think of how you can ‘give’ to others; rather than thinking about your own circumstances. Have your Holiday Time be about ‘giving’, instead of ‘receiving’, and you will notice a change in your Mindset that can change your life. Begin to look at each moment, each experience, and each person as if “they are brilliant and perfect”. Look at the people who show up in your life, as if they are brilliant and perfect. Focus on the positive possibility rather than on the negative expectation. You will notice when you do that, that people actually change, and you have more fulfilling and satisfying experiences.” And remember: “Life is a joke – you just have to stand back far enough to see the punch line! Lighten up. Go with the flow. Focus on the possibility, instead of the obstacle.” Marcia Martin www.marciamartin.com marcia@marciamartin.com Photographic Acknowledgments: Photography - Carl Studna www.carlstudna.com Photography Design - Brooks Cole www.holocosmos.com (photographs supplied by Marcia Martin)
